Stand alone Mailchimp subscribe and unsubscribe form

We need the ability to have a stand alone Mailchimp subscribe and unsubscribe form. When I attempt to create a page I get form errors clashing with the WHMCS software. Some clients want to see a signup form without needing to signup a a client yet.
